Under a $3 million program, Washington State is doubling its incident response trucks (IRTs) from 19 to 38, beginning in July 2002. Washingtons legislature authorized the Washington State Department of Transportation to borrow money from the States paving program to expand the incident-response team. Since January 1, 2002, the States IRTs have responded to nearly 3,000 incidents. The sooner an IRT can get to an incident, the sooner the traffic can flow once again.

Small banks with assets of under $1 billion hold 24 percent of the banking industrys assets, but provide almost half of the small business lending.Smaller banks, each with assets less than $300 million, do 35 percent of small commercial lending, even though they account for only 15 percent of total U.S. banking assets.

According to Paul Johnson of the North Carolina High Point Enterprise, the USBIC data show that during an eight-year post-NAFTA period, the level of import penetration at least doubled in 26 industry sectors. By 2005, 24 industries had lost 50 percent or more of their U.S. market to imports? For small businesses, NAFTA has meant both the loss of existing clients and future income opportunities. And because many Americans depend on small businesses for jobs, the NAFTA hit on small business means hard times for the middle class.

The increase in construction activity was higher in civil engineering. According to seasonally adjusted data, the value of construction put in place in civil engineering rose substantially for the fourth consecutive quarter (by 9.8% in Q1 alone) and was 67.4% higher than in the same quarter of 2006. This vigorous growth is estimated to reflect the accelerated construction of motorways. Activity in the construction of buildings declined according to seasonally adjusted data. The value of construction put in place in the construction of buildings rose steadily from quarter to quarter last year to reach a record-high level in Q4. The value of construction put in place decreased by 2.0% in the three months to March this year but was still 21.0% higher than in the same period last year. Seasonally adjusted activity declined both in the construction of residential and nonresidential buildings. Otherwise, activity rose considerably in the construction of non-residential buildings while it fell in the construction of residential buildings compared with the same period of 2006. In the latter, it even reached the lowest level in two years, according to seasonally adjusted data. In interpreting the value of residential construction put in place we note that these figures exclude the activity of smaller companies (see the note under the table) in which, in our estimate, the construction of buildings is the main activity. The increase in the number of employees continued in Q1. The number of people employed in the construction sector in 2006 was 7.5% higher than in the year before. In the first quarter of 2007, the number of employees rose by as much as 11.3% over the year before. The number of employees by activity rose particularly in building completion and general construction, similarly as last year. In terms of employee status, sole proprietors and their employees recorded higher growth rates (also see p. 10).

Loans are an important part of your business. You want to encourage your loan officers to generate increasing numbers of quality loans for your credit union. What can you do to boost performance, raise the numbers and invigorate your staff? Many credit unions turn to incentives. * A flat, per-file incentive. “I like it because its easy to track, but I dont like it because it doesnt reward them for selling up a higher loan amount. * A percent of the average yield. “This ensures they dont give away rate if you allow some rate matching flexibility or if they can barter buying down rate with points. The only problem with this is that they might try to sell a higher rate to get more incentive and that isnt in the members best interest. If we give loan officers some discretion to match a competitors rate, the loan officer might be tempted to always reduce our rate and give the member a lower rate all the time rather than only when they need to match a rate. If we pay incentives based on the average yield, they would be affecting their incentive by matching rates all the time. This structure would ensure that they offer our posted rates and only match rates when there really is a competitive offer.”

Southwestern Bank and Trust Co. plans to open a loan production office in the Waterford Office Complex this week to boost its presence in north Oklahoma City. That branch “has actually been very successful, and thats the reason for opening this Waterford office — to give us more of a presence in north Oklahoma City,” said Thomas W. Parrish, president and chief executive officer of the bank. Southwestern Bank “has a number of customers out in that area. Our MegaMarket branch was designed to accommodate the loans and deposit needs in that area of Oklahoma City. A company in California custom-made the $80,000 vehicle that Hollar claims is the only one of its kind in the Oklahoma City area. Weve only had it for about 60 days. And its leased out at least every Friday and Saturday night and often during the week.

Irritable bowel syndrome (IBS) is common, with each UK GP seeing an average of about eight patients every week. It is unpleasant for sufferers, negatively affects quality of life, and is expensive for health services. A large proportion of patients do not do well with conventional therapy, and many seek unconventional alternatives. One of these is a form of hypnotherapy known as gut-directed hypnotherapy. It is based on relaxation to try to normalise gut function. Because there are claims that it works, some purchasers are tempted to provide a service. A systematic review of trials [1] suggests a large degree of caution is warranted. Authors sought studies, of any design, in nine electronic databases, and even contacted authors for information about any further studies.
Hypnosis is widely used and accepted as an adjunct to treatment for pain control, habit disorders, symptom reduction, and many medical problems. Although in the past the application and effectiveness of hypnosis have been questioned by the medical community, the clinical disciplines that use hypnotherapy have grown to include the medical specialties, psychiatry, psychology, and social work. In the last 20 years over 100 review articles have been published by a diverse membership in both the American Journal of Clinical Hypnosis and the Journal of Clinical and Experimental Hypnosis, which are the respective journals of the American Society of Clinical Hypnosis and the Society of Clinical and Experimental Hypnosis. The International Society of Hypnosis is composed of individual members from more than 20 national constituent societies. The use of hypnosis is widespread in various areas of clinical medicine, and family physicians are in an ideal position to address questions about the indications, misconceptions, and contraindications for the use of hypnosis. The physicians understanding of the history, theory, and clinical application can help guide the patient to this alternate therapeutic modality. Education in basic and advanced hypnosis can also enhance the family physicians armamentarium in diagnosing and treating many common ailments.

AN ITALIAN who claims to be a qualified criminal lawyer was asked yesterday to explain himself to three Court of Appeal judges. Giovanni di Stefano, who has represented some of Britains most notorious criminals, including Kenneth Noye, Jeremy Bamber and Harold Shipman, was questioned by the court over his right to practise law in Britain. Mr Lynch replied: “Mr Di Stefano. I dont believe he is a representative of Paul Martin.” But the judge said that Mr Di Stefano, who said he worked for Italian law firm Studio Legale Internazionale, had signed written legal advice in connection with the appeal.
THE SOCIETY of Black Lawyers yesterday demanded the setting up of a new race-hate register so that racist criminals could be more closely monitored by the police. Peter Herbert, the societys chairman, said research had shown that racist attacks were rarely spur of the moment incidents and were often carried out by perpetrators from identifiable problem estates or families. He said because there were identifiable patterns, resources could be targeted. The society wants to see the subject of racism introduced into the national curriculum for primary schools. Mr Herbert said this month had been dedicated “Black History Month” yet only a handful of schools were taking part in the exercise. The Society of Black Lawyers is organising the first international conference on race-hate crimes, to be held in London in December. The high-profile black American lawyer Johnnie Cochran and the United States deputy attorney general, Eric Holder, will attend along with Stephen Lawrences parents and their lawyers. Mr Herbert said the evidence of bad practice which has emerged at the Lawrence inquiry had been received with dismay in the US, where Scotland Yard has legendary status. “The British police and Scotland Yard have a reputation for dogged thoroughness and it is something that has tarnished their international reputation,” he said. A total of 91 women, aged 30-69 years, participated in the study; 50 were nonsmokers and 41 were smokers. The subjects completed questionnaires on smoking status and dietary habits and provided blood samples for nutrient analysis. Serum levels of [alpha]-carotene, [beta]-carotene, cryptoxanthin, and lycopene in smokers averaged only 71-79% of the concentrations found in nonsmokers. Dietary differences could not account for this effect. Mean serum concentrations of vitamins C and E and lutein/zeaxanthin were only slightly, and not significantly, lower among smokers, and concentrations of vitamin A were actually higher. The results for carotenoids are generally consistent with those of other studies. The lack of an association between smoking and serum vitamin C levels may reflect the relatively high vitamin C intake of this study population. The authors observe that the negative effect of smoking on serum concentrations of antioxidant carotenoids may pose a serious health risk in low-income populations already at higher risk for many chronic diseases.
This study, performed in Bonn, Germany, evaluated the effect of four weeks of supplementation with folic acid (400 [micro] g/day), vitamin [B.sub.6], (2 mg/day), or both on homocysteine levels in 106 healthy young women. As compared to women receiving placebo, those receiving folic acid, with or without vitamin [B.sub.6], showed a significant reduction in plasma homocysteine levels (17% in the group receiving both vitamins, 11.5% in the group receiving folic acid alone, with no significant difference between these two groups). Vitamin [B.sub.6], administered alone, had no significant effect on homocysteine levels. The results of this study indicate that supplementation with physiologic doses of folic acid can reduce homocysteine levels within the normal range. Since even moderate elevations in homocysteine concentrations are a risk factor for cardiovascular disease, the reduction in homocysteine produced by supplementation might have long-term health benefits. Additional studies are needed to determine whether similar effects on homocysteine can be produced with dietary folate rather than folic acid supplements. Southwestern Bell is hoping to meet the needs of small and home- based businesses with a new product designed to allow customers to use their computers like a telephone. The company Tuesday announced it has launched its new software, PC Phone Manager, in Oklahoma to offer its customers a way to integrate computers and telephone service. This marks the companys entry into the growing computer telephony integration market — an industry expected to account for $3.3 billion in 1996, according to Rick Wilson, product manager for Southwestern Bell.

This “picture of health” has been produced and refined by my 10- year diet of the front covers of womens health magazines featuring beautiful young women with their flat stomachs and tanned bodies. “There are [more than] 30 million Americans who have no health insurance and another 20 million who have inadequate health insurance,” says Rep. Louis Stokes (D0Ohio), chairman of the Congressional Black Caucus Health Brain Trust. “It is mandatory that we begin to try and give Americans–particularly minorities–adequate health care as a right.” The Bush plan would give people not covered by Medicaid tax credits, and families making less than $80,000 a year tax deductions to pay for medical costs. He would also pass laws to ensure that even chronically ill patients could receive health insurance. The plan has received a cool reception on Capitol Hill, and alternate proposals, including a full-scale, government-paid national health insurance plan, are under development. But what does America really want for its health care system? The views are many.
The national survey of more than 2,000 adults was conducted by the Kaiser Family Foundation and the Agency for Health Care Research and Quality The survey also showed that Americans rely on the recommendations of friends and family more than standardized quality indicators when selecting a health care provider, and people have little confidence that employers will give reliable information about the quality of health care providers. And despite the undeniable draw of the Internet, people still trust doctors and pharmacists more than advertisements and the Web for information on prescription medications, the survey shows.

Perfumes have played an essential part in the lives of t he people of the Middle East for thousands of years. In markets as far afield as the Khan el Khalili in the old city of Cairo to the five acre site of covered stalls in the ancient walled city of Kano in northern Nigeria the odour of attar of roses, sweet jasmine and the spicier ambience created by oud, frankincense and sandalwood, encapsulate the very spirit of Islamic culture. The perfumes of Arabia represent the vibrant texture of Islamic life, with all its religious, medical and scientific ramifications. So, although these exotic fragrances are prized for their obviously sensuous and heady allure, their hold over such a widely disparate public speaks volumes of a long history of use, with functions that go deeper than merely whispering improbable promises. One of the earliest fragrances of which we remain aware was used by the ancient Egyptians about 4,000 years ago. This is myrrh, a thick, yellow, gummy resin extracted from a shrub found in Dhofar in southern Oman, in Somalia and Ethiopia.
EVER WALKED away from a perfume counter trailing a haze of woody, citrusy and floral smells, with mysterious phrases such as “top notes” and “base notes” ringing in your ears? Useful knowledge when selecting a gift in a department store perfume hall five minutes before closing on Christmas Eve: if you know which family of scent your intended enjoys, then choose one from the same family. In the last column (Medical Malpractice: The First Stage, The Physician Executive, January-February, 2008) I discussed the importance of proper documentation to avoid a lawsuit or gain an edge if one is filled. Now, assume your best efforts failed and a lawsuit is under way. I cannot describe the feeling of opening an envelope with your name on a long legal document with the words: breach. standard of care. damages. wrongful death written in black and white. Your first instinct is to vomit. Now begins the discovery phase of the litigation process, where the plaintiff and defense have an opportunity to test the strength of their cases by taking depositions and submitting evidence to be used at trial.

The authors note that there have been 13 studies which used prospective randomized control designs which compared hypnosis either to baseline level noiception or to a control condition of some sort. These studies consistently demonstrate that hypnosis can assist patients with chronic pain with a reduction in their pain experience and pain related coping for a variety of chronic pain conditions. Hypnosis was generally found to be more effective than other nonpharmacological interventions such as physical therapy, attention, and psychoeducative strategies. The intervention which was most commonly employed was training in self-hypnosis. The authors provide a critique of these studies in that there appears to be a lack of standardization in the uses of hypnotic interventions in these clinical trials and also that the treatment group numbers tended to be lower than one might desire. The authors also provide a number of very useful suggestions for future research and clinical application of hypnosis to assisting patients with chronic pain syndromes. In the language of the laterality of brain functions, cognitive therapy has emphasized “left brain” thinking as opposed to “right brain” creativity and imagination. In Cognitive Hypnotherapy, Tom Dowd successfully changes this emphasis-adding “right brain” to what was mostly a “left brain” art. Technology, naturally, is to blame. The simultaneous arrival of the mobile phone and laptop means theres no escape. The only solution for hard-pressed senior executives is to switch their phones off and claim they are in a location too remote for a signal. When I called San Francisco, I was referred back to Charlotte and after three days of phone-tag with a spokeswoman (not totally her fault), she referred me to a Web site covering the Uniform Commercial Code, which supposedly explains the now-famous rules and regulations. The spokeswoman didnt seem to know how to access the Web site, so told me to call Google.

This can be done with common stock dividends, preferred stock dividends, or stock repurchase by the business of its own stock. The aim of this decision is to retain the resources in the business that are required to run the business or make additional investments in the business, as long as the returns earned exceed the required return. In theory, management should return or distribute all resources that cannot be invested in the business at levels in excess of the required return. In practice, however, dividends are often maintained at or changed to certain levels in order to convey the proper signals to the investors and the financial markets. For example, dividends can be maintained at moderate levels to demonstrate stability, maintained at or reduced to low levels to demonstrate the growth opportunities for the business, or increased to higher levels to demonstrate the restoration of a strong financial (capital) structure (debt and equity capital) for the business.Capital is the total of financial resources invested in the business. In producing some two dozen multi-projector slide shows and a dozen video presentations on our travels my wife and I have learned some valuable pointers about making successful travelogs. First, we find video to be most fascinating to use, easier to present and very much more interesting to an audience. Second, we must give sufficient attention to all three stages of the process: preparation before departure; taping during the journey; postproduction editing after you get home. Study all the information you can obtain about the places to be visited. For USA motor travel, the AAA and State Departments of Tourism are prime sources. For PSA members the “Travel Aids” listed by the Photo-Travel Division Directory can be quite useful. For foreign travel your travel agent can get all sorts of current literature and information including the addresses of Tourist Travel Bureaus maintained in the US by most foreign nations. We have found Fodors Guides most useful in many parts of the world and Michelin Guides good for Europe. Use the information to plan what locations to tape and how to reach them.

The Canadian portion of the natural gas Alliance Pipeline, which is now under construction, has a mainline lineage of 1,656 km plus several hundred more kilometers of lateral feeder lines. The Alliance Pipeline Limited Partnership of Calgary, Alberta, Canada is the company that will operate this section of pipeline. The engineering company responsible for routing, design and construction is Singleton Associated Engineering Ltd. (SAEL)–also of Calgary. Houston-headquartered and Saskatoon, SK Canada-based Petris Technology Inc. was chosen, based on its extensive North American and overseas experience, to perform the aerial videography. The pipeline corridor had been previously selected by multiple-manned (SAEL) airborne exploratory flights and supporting environmental, landowner, existing pipeline and many other data. SAEL had also captured excellent, high-level, black-and-white aerial photography of the right-of-way for alignment sheet production. Note: The sign-up process shows users confusing options — but they can be ignored. Just stay with the defaults provided, unless you know you want something else. The purpose of this e-mail list is to give patients, industry, academia, other government agencies and other interested parties one source for FDA HIV/AIDS related information. Information such as product approvals, significant labeling changes, safety warnings, notices of upcoming public meetings and alerts to proposed regulatory guidances for comment will be distributed through this e-mail list.

Cement use in the historic cemeteries of the French Quarter Although most concrete crypts and mausoleums are built in low-lying areas, where water could easily destroy a coffin, its not entirely true for the city of New Orleans. Basically, the majority, if not all, of the crypts and mausoleums in the cemeteries of New Orleans were built along the lines of traditional European cemeteries and to represent the architecture of a French city. In fact, many visitors to New Orleans have stated that these cemeteries do look like a miniature city-a “city of the dead”1 (Fig. 2 and 3). Most of the crypts and mausoleums in New Orleans are constructed of brick covered with a cement plaster (see Fig. 4). Even when these crypts and mausoleums need repair work done, workers use a mixture of both lime and cement. This combination does not detract from the original structure.2
No retrospective would be complete without remembering the contributions of leading figures who have helped shape the industry. In this issue recognizing todays top contractors (see page 27), it is fitting to salute some of the pioneers from CONCRETE CONSTRUCTIONS early days. It was Corbettas dedication to the development of a strong concrete industry, however, that earned him the moniker Mr. Concrete. He was instrumental in founding several organizations, starting in 1950 with the Concrete Industry Board of New York City, which brought together concrete contractors, producers, architects, engineers, testing labs, cement manufacturers, and reinforcing steel producers.
